Stroke Services Phase 3 Bid
The Modernisation Initiative Stroke Services Programme has been awarded £3.4 million by Guy’s and St Thomas’ Charity to continue its programme of service improvement and radical change for people living with stroke in Lambeth and Southwark until December 2007.
The Modernisation Initiative Stroke Services programme is a major three year project working across the healthcare sector in Lambeth and Southwark to identify and deliver change in patient centred care. The project works in partnership with local modernisation change agents, including NHS staff and patients, numerous community groups, patient groups and independent healthcare organisations.
In addition to maintaining and extending the existing programme of work, the additional funding will resource a radical programme of improvement in community services used by people who have had a stroke, support service users learn to live with a disability, and significantly enhance the level of stroke skills in the local workforce.
The focus will be to streamline the pathway in current service provision in Lambeth and Southwark PCTs, enhance the levels of specialist knowledge in existing services by supporting development of existing staff, and support organisational and modernisation programmes to develop best practice.
The work will transform hospital discharge, redesigning the service to enable local service users to leave hospital at the most suitable time. Hospital discharge will be backed up by an enhanced community rehabilitation service. Immediately upon discharge, local service users will be able to access intensive rehabilitation within the community at an appropriate level.
Specialist capacity and competence is to be improved. Expert practitioners will support the work of existing staff and contribute to their overall professional development. Competency based training and development will be implemented for all staff working with people who have had stroke. In this way, we can provide not only an enhanced clinical support service, but a strong learning culture where developing skills and knowledge can become quickly embedded.
In addition, a Long Term Support Network is being tested for people living with a stroke in Lambeth and Southwark. The network will help people regain their confidence after having a stroke, put them in touch with others in a similar situation and provide support to help them access opportunities such as returning to employment or study, and restarting hobbies or leisure activities.
